Dementia Toolkit

Provides information and guidance to support palliative care and advance care planning for people living with dementia and their families and carers.

The ELDAC Dementia Toolkit is designed for clinicians and care workers providing palliative care and supporting advance care planning for older Australians living with dementia, their families and carers. The ELDAC Dementia Toolkit is user-friendly and has been developed and reviewed by experts in aged care and palliative care.

The ELDAC Dementia Toolkit has evidence-based resources on palliative care and advance care planning that can assist you and your organisation in meeting the strengthened Aged Care Quality Standards.

The toolkit is divided into three main sections:

  • Clinical Care
  • Assess Your Knowledge
  • Practice Exemplars.

Some features of the toolkit include:

  • Clinical assessments, guidance and information to inform palliative care and advance care planning when caring for people living with dementia.
  • Case studies for individual and collective reflection on caring for people living with dementia and their families and carers.
  • Resources for clinicians and care workers, such as on providing grief and bereavement support.
  • A Dementia Knowledge Assessment Tool (DKAT2) and ELDAC Dementia Personal Learning Plan.
  • Practice exemplars on providing palliative and end-of-life care for people living with dementia.
