Palliative care needs are highly complex and evolve quickly, requiring flexible models of care that embrace a multidisciplinary team-based approach across diverse care settings.

Primary care professionals are well placed to both provide and coordinate palliative and end-of-life care for older people. Using the ELDAC Care Model as a pathway to understand the needs of older people with palliative care needs, in this section of the toolkit you can find the clinical tools, information, and resources to support you in the delivery of care.
